Hospital readmissions are frequent, harmful and costly - and are about 23% of Medicare patients. Over 2.3 million patients annually are rehospitalized within 30 days after discharge, incurring a cost of $17 Billion, which constitutes nearly 20 % of Medicare’s total payment. 
​          It is reported by the Medicare Payment Advisory Commission (MedPAC) that about 75 % of such readmissions can and should be avoided.
